Health care reform (ie, the "Affordable Care Act") mandates that most plans provide benefits not previously offered. These are expensive benefits and we project a $300-500 per employee per year increase in the "grandfathered" health plans we manage. Those plans that are not grandfathered will see even higher costs. Medical services are the obvious drivers of increasing health plan costs, and for self-funded health plans usually represent 70% of the total plan costs. Thus, if a plan is to control its cost, the major emphasis must be on the medical services.

About 10 years ago, Dr. David Herbert was on the verge of retirement from a long and successful career in medicine in Texas. His friend, Michael Jenike, who also had a long career in hospital administration and managed care, was about to do the same. They had a mutual friend on the hospital board who owned a large steel fabricating company who was concerned about his spiraling health care costs for his 1,200 employees and their dependents. He asked Dr. Herbert and Michael if they could help. They did help and the rest is history.

In 1999, a large employer asked Michael Jenike to take a look at the company's self-funded health plan to see whether anything could be done to reverse the alarming, upward cost trend. At the time, Jenike was in the business of running profitable hospitals and therefore, intimately involved in negotiating network rate contracts with national carriers and Third Party Administrators who offered Managed Care products.

Consumerism is good, Right?
I tried to be a good health care consumer recently. Here's my story... My 14 year old daughter recently had a biopsy on a suspicious spot on a birthmark. The Pathology report indicated Basel Cell Carcinoma. That was good news because Basal Cell is usually a non-malignant, slow growing cancer.
